Prepare for Medication Safety Systems Accreditation

A step-by-step, non-scored preparation path that shows applicants what to organize, what reviewers will trace and how scope is defined.

Preparation sequence

Seven steps from scope definition to an audit-ready evidence file

1

Define the proposed scope

A defined medication-use safety system, care setting, service line or organizational operating unit approved in scope.

2

Read the standard stack

Use CHSPS-CORE-100 + CHSPS-MSS-310. Mark every requirement as applicable, not applicable with rationale, or needing evidence.

3

Build the controlled evidence index

Link each applicable requirement to the authoritative policy, record, report, system output or sample that demonstrates implementation.

4

Test implementation with tracers

Trace real operations such as high-risk medication safeguard and medication reconciliation case.

5

Close material gaps

Correct the immediate condition, identify cause, assign corrective action and verify effectiveness when risk or recurrence warrants it.

6

Complete internal management review

Confirm scope, unresolved risk, evidence integrity, staffing, vendors, changes and readiness for independent review.

7

Submit the eligibility and scope inquiry

CHSPS confirms fit and scope before formal application and secure evidence submission.

No predictive pass score. CHSPS accreditation decisions are requirement- and evidence-based. A single unresolved high-risk control failure can matter more than many complete low-risk records.
CHSPS-MSS-310

Organize evidence around the published standard

The cards below summarize the controlled standard structure. The PDF and machine record remain authoritative.

01

Safety governance

Show the medication-safety governance structure, accountable leaders, committee or review mechanisms, escalation authority, risk priorities, action tracking and evidence that safety decisions are implemented across the defined scope.

02

Risk assessment

Show how medication-use risks are identified and prioritized, data and event sources considered, responsible reviewers, risk-control decisions, reassessment triggers and completed mitigation actions.

03

Transitions

Show medication reconciliation and transition-of-care controls, responsible roles, source reconciliation, discrepancy resolution, communication at transfer or discharge and representative completed cases.

04

High-alert medications

Show the defined high-alert medication list or equivalent risk classification, safeguards, independent or technological checks where used, storage and administration controls, exceptions and monitoring of safeguard performance.

05

Prescribing & CDS

Show prescribing controls and clinical decision support governance, alert or rule design, approval, testing, override monitoring, change control and representative evidence that intended safeguards are active.

06

Verification

Show pharmacist or other required verification workflows, role authorization, required checks, exception handling, queue or turnaround monitoring and representative records linking the verification to the medication order.

07

Preparation & storage

Show preparation, labeling, storage, segregation, environmental or stability controls as applicable, inspections, discrepancy handling and representative records demonstrating controlled medication handling.

08

Administration

Show patient, medication, dose, route and timing controls as applicable to scope, barcode or other technology use, independent checks where required, exception workflows and representative administration records.

09

Monitoring

Show patient and therapy monitoring requirements, trigger thresholds, responsible roles, escalation, follow-up, documentation and representative cases where monitoring changed or confirmed medication management.

10

Learning system

Show event and near-miss reporting, triage, investigation, causal analysis, action assignment, communication, effectiveness review and trend analysis demonstrating that reported safety signals lead to learning.

11

Technology

Show governance of medication technologies, configuration ownership, testing, access control, downtime procedures, interface or data-quality monitoring, change management and review of technology-related events.

12

Supply resilience

Show shortage, recall and supply-disruption controls, risk assessment, therapeutic or operational alternatives, communication, inventory actions, approval and monitoring of temporary safeguards.

13

Diversion interface

Show how medication-safety processes interface with diversion prevention and response, including discrepancy escalation, access review, investigation coordination, patient-safety assessment and documented follow-up.

14

Workforce competence

Show role-specific medication-safety competencies, initial and continuing assessment, high-risk skill controls, overdue or failed competency response and evidence that authorized staff remain current.

15

Patient partnership

Show how patients or caregivers receive and contribute medication information, how discrepancies and concerns are captured, accessibility or language needs are addressed and safety feedback is incorporated.

16

Performance

Show medication-safety measures, event and near-miss trends, denominator and data-source controls where applicable, management review, improvement priorities and evidence that interventions are evaluated for effectiveness.

Implementation review

Expect reviewers to trace real operations, not only policies.

Representative tracerWhat to be ready to show
high-risk medication safeguardTrace one high-risk medication from order or request through verification, preparation or dispensing, administration and monitoring, showing each required safeguard, any override and the retained evidence.
medication reconciliation caseTrace one transition case through medication-history collection, source comparison, discrepancy identification, clarification, reconciliation, communication and final documented medication list.
safety event to CAPATrace one medication-safety event or near miss from report through triage, investigation, causal analysis, immediate risk control, corrective or preventive action, effectiveness review and closure.
recall or urgent safety communicationTrace one recall, shortage or urgent safety notice from receipt through impact assessment, inventory or therapy action, communication, patient or prescriber follow-up where applicable and documented closure.
