CHSPS-ACT-800 · Public accountability register

Adverse accreditation actions should be as visible as accreditation awards.

This register publishes CHSPS accreditation actions that materially change, restrict or restore a public accreditation status. Use it together with exact-number verification: the verification record remains the authoritative current status for a specific accreditation.

Verification

What is this organization's current status?

Use the exact accreditation-number lookup. Verification is the authoritative status record for accredited, suspended, withdrawn, expired and other public lifecycle states.

Public actions

Has CHSPS taken an adverse or restorative action?

This register explains public actions that materially change, restrict or restore accreditation status and the basis/effective date of the action.

Controlled vocabulary

What does the status actually mean?

CHSPS-ACC-101 controls the decision and status terms. This page does not create a parallel vocabulary.

Current public actions

The register is currently empty.

As of this August 28, 2026 register review, no public adverse or restorative accreditation actions are listed as in effect.

No current entries

If a publishable action becomes effective, CHSPS will identify the affected record, action, effective date and governing basis here and reflect the publishable status in the organization's verification record.

Check a specific record

Important: absence from this register is not a substitute for verification. If you are evaluating a specific organization, always use the accreditation number in Verify Accreditation.

What gets published

Actions that materially affect a public accreditation record.

The public-action register is narrower than the complete accreditation lifecycle. Routine awards and ordinary good-standing renewals belong in verification, not in an adverse-action register.

Suspended

A previously accredited program temporarily loses some or all accreditation rights pending corrective action or another controlled disposition.

Public record effect
Public verification shows the suspended status when publishable.
Mark / claim effect
Use of the CHSPS accreditation mark and related claims is restricted in accordance with the suspension and mark policy.

Withdrawn — for cause

CHSPS terminates accreditation because of sustained or serious nonconformity after the applicable controlled process.

Public record effect
The historical verification record reflects the for-cause termination.
Mark / claim effect
Accreditation mark and current-accreditation claims end.

Expired with unresolved public claims

The accreditation term has ended and the organization continues to present or imply current CHSPS accreditation.

Public record effect
The verification record remains historical/expired; a public action may be used when continued claims create a public-record concern.
Mark / claim effect
Mark rights end at expiration.

Reinstatement after an action

A new authorized CHSPS decision restores accreditation following correction or another qualifying route.

Public record effect
The restorative action is published with the same public-record discipline as the action it closes.
Mark / claim effect
Rights resume only to the extent and from the effective date established by the new decision.

Denied decision upheld where status is still claimed

Initial accreditation was not granted and the adverse decision is final within the applicable CHSPS process, yet the organization continues to represent itself as accredited.

Public record effect
The public record can clarify that no CHSPS accreditation was granted.
Mark / claim effect
No CHSPS accreditation mark rights arise from applicant or denied status.
Normally not listed here as adverse actions

Initial awards, ordinary timely renewals, routine good-standing maintenance, applicant status, and voluntary non-renewal without outstanding public claims are handled through the appropriate verification/lifecycle record rather than being presented as adverse actions.

Minimum public record

A published action should answer seven questions.

The public should not have to infer what happened from a vague status label.

1. Record holder

The exact legal/program name associated with the accreditation record.

2. Accreditation number

The identifier that can be reconciled to the public verification record.

3. Program / scope

The affected accreditation pathway and scope; an action should not be read more broadly than the record supports.

4. Action / status

The controlled action or resulting public status, using CHSPS-ACC-101 terminology where applicable.

5. Effective date

The date on which the public action or resulting status takes effect.

6. Governing basis

The controlled standard, policy, procedure or decision basis supporting the action.

7. Current resolution

Whether the action remains in effect, was superseded, or was closed by reinstatement or another final disposition.

How an action becomes public

Decision first. Due process next. Publication follows the controlled status.

1

Documented basis

The case record identifies the applicable standard or controlled obligation, findings, responses and relevant procedural record.

2

Independent decision

The authorized Accreditation Decision Committee acts under CHSPS-ACC-108. Technical reviewers do not issue the final accreditation decision on the same file.

3

Written notice and rights

The organization receives the decision, rationale and applicable reconsideration/appeal route under CHSPS-ACC-101 and CHSPS-ACC-102.

4

Public-status treatment

A filing does not itself change public status during review except where required to protect the public. Publication follows the status that is eligible for public treatment under the controlled policies.

5

Verification reconciliation

The affected accreditation record and this public-action register should tell a consistent public story once the action is publishable.

Checking a specific organization

Do not use this page as a substitute for exact-number verification.

This register answers whether a public adverse/restorative action is listed. Verification answers the current status of a specific CHSPS record. A claim that does not verify should be reported even if the organization does not appear in this register.
