For hospitals, health systems and pharmacies — independent review of how your medication-safety system governs, prevents, detects and learns from medication risk, not just how it is written down.

Healthcare quality professionals reviewing medication-safety evidence, performance measures and corrective actions
Fit and next steps

Who it’s for, and how it works

A good fit

  • Hospitals and health systems
  • Pharmacies and medication-management organizations
  • Defined clinical service lines with an operating medication-safety system

Not the right fit

  • Workforce competency systems — see WCA
  • Clinical education quality — see CEQ
  • Individual clinicians, pharmacy licenses or medication products — these are outside MSS scope
  1. Free eligibility reviewAbout 5 minutes to submit. CHSPS sends a written outcome, targeted within 10 business days after a complete inquiry.
  2. Scope & agreementCHSPS confirms the scope in writing. You sign the applicant agreement and pay the $1,250 application fee, credited toward the $5,500 total.
  3. Evidence & technical reviewYou submit evidence and the $4,250 review balance. Conflict-screened reviewers evaluate it, and you can answer any findings.
  4. Independent decisionA separate committee makes the decision, and accredited scope becomes publicly verifiable. The review timeline is stated in writing at scope confirmation, before you pay anything.
STEP 1 · CONFIRM FIT

Choose MSS when the medication-safety management system is what you want independently reviewed.

MSS is designed for hospitals, health systems, pharmacies and medication-management organizations accountable for defined medication-use systems.

STEP 2 · DEFINE SCOPE

Know exactly what MSS accreditation covers.

Scope is explicit. Accreditation applies only to the defined medication-use safety system, care setting, service line or operating unit approved in the CHSPS record.

What is accredited

The medication-safety management system

Governance, safeguards, reconciliation, event learning, competency, recalls, corrective action and improvement within the approved scope.

Functions in scope

The medication-use process and its controls

Prescribing, verification, preparation, storage, administration, monitoring, learning, technology and resilience controls applicable to the defined system.

What is outside scope

No blanket healthcare-organization claim

Individual clinicians, pharmacy licenses, medication products, unrelated services and organization-wide hospital or regulatory accreditation are not implied.

OPERATIONAL AUDIT

The review traces the medication-use system, not only the policy set.

Technical review combines documents with source evidence, interviews, demonstrations and end-to-end tracers.

Trace a high-alert medication from order through verification, dispensing, administration and monitoring.
Select a serious medication event and follow it through analysis, corrective action, effectiveness review and learning dissemination.
Trace a critical recall or shortage from notification through inventory control, substitution and clinical communication.
STEP 4 · ELIGIBILITY & FEES

Confirm operating maturity before a paid application begins.

The free review confirms MSS fit and proposed scope before CHSPS starts a paid engagement.

STEP 5 · ACCREDITATION PROCESS

What happens after eligibility.

MSS uses the same independent seven-stage accreditation architecture used across CHSPS pathways.

  1. 01Free Eligibility Review

Confirm MSS fit and proposed medication-safety scope.

  2. 02Scope & Agreement

Receive written scope confirmation and execute the applicant agreement.

  3. 03Self-Study & Evidence

Submit governance, high-alert, CDS, incident, CAPA, competency and performance evidence.

  4. 04Technical Review

    Conflict-screened reviewers evaluate CORE-100 + MSS-310 evidence and tracers.

  5. 05Findings & Corrective Action

Resolve findings and provide corrective evidence where required.

  6. 06Independent Decision

A separate decision body determines accreditation status.

  7. 07Public Verification & Maintenance

Approved scope, dates and status become publicly verifiable.

STEP 6 · VALUE

What MSS accreditation gives you.

Independent assurance focused on the medication-safety controls that matter when patient safety, quality or governance is challenged.

01

Independent system review

External evaluation of the defined medication-safety management system beyond internal quality assurance.

02

System-wide traceability

Review spanning prescribing, verification, preparation, administration, monitoring and learning.

03

Improvement-system assurance

Evidence that events lead to analysis, corrective action, effectiveness review and organizational learning.

04

Public verification

A CHSPS record identifying the exact accredited medication-safety scope and current status.

BEFORE YOU DECIDE

STEP 7 · AFTER ACCREDITATION

Maintaining MSS accreditation.

MSS remains a continuing relationship throughout the three-year term. ACC-109 governs the common maintenance cycle.

Annual cycle

Keep the record current

Annual reporting may address:

  • medication-safety activity and significant-event trends
  • high-alert controls and medication-use process changes
  • CDS, verification, preparation, administration and monitoring
  • learning-system activity, supply resilience and diversion interface
  • workforce competence and patient partnership
When things change

Report material changes

Examples include:

  • medication-use system scope or governance
  • high-alert safeguards or clinical technology
  • preparation, storage, administration or monitoring model
  • supply resilience or material vendor arrangements
  • diversion interface or workforce-competence system
Renewal

Revalidate the living system

Renewal revalidates:

  • safety governance and risk assessment
  • high-alert and medication-use process controls
  • technology, monitoring and learning-system evidence
  • supply resilience, workforce competence and patient partnership
  • performance monitoring and sustained improvement

Questions buyers ask first

What does it cost, all-in?

$5,500 for the initial specialty review: the $1,250 application fee when you sign the applicant agreement, then the $4,250 review balance when you submit evidence. After that, $1,500 a year and a $3,500 renewal review every 3 years. A concurrent additional specialty scope in the same organization is $2,750. The eligibility review is free.

How long does it take?

The free eligibility review targets a written outcome within 10 business days after a complete inquiry. CHSPS does not publish a single planning target for this pathway; the review timeline is stated in writing at scope confirmation, before you pay anything.

What do we need to start, and what evidence comes later?

Nothing to upload. Step 1 asks only for your name, email and organization; the rest of the form covers scope, operating status and readiness, in about 5 minutes. Formal evidence comes after the agreement is signed: governance, high-alert medication, clinical decision support, incident, corrective-action, competency and performance evidence.

Can organizations outside the United States apply?

Yes. CHSPS is a United States–based 501(c)(3) nonprofit; organizations in any country may apply.

What if we are not eligible?

Every eligibility review ends in a written outcome: Eligible, Potentially Eligible — Additional Information Required, or Not Eligible, with the specific thresholds not yet met. There is no charge for the review, and if a different CHSPS pathway fits better, the review says so.

READY TO START?
