Purpose
To ensure that CHSPS standards, policies, procedures and other authoritative publications are identifiable, approved, current, traceable and available to the people who rely on them.
1. Scope
1.1 This policy applies to controlled standards, accreditation frameworks, governance and accreditation policies, formal procedures, program requirements, decision vocabularies, fee schedules, methodologies, specifications and other documents designated by CHSPS as controlled.
1.2 Marketing copy, ordinary news items and temporary working drafts are not controlled documents unless expressly designated as such; they shall not override a controlled document.
2. Required control information
2.1 Each controlled document shall identify, as applicable: document ID, title, edition or revision, status, approving authority, approval date, effective date, owner, superseded edition and next review date.
2.2 Drafts shall be plainly labeled as drafts and shall not be represented as effective requirements. Public drafts shall state the review status and comment window.
3. Authority and approval
3.1 The document owner is responsible for technical accuracy and initiating review. Approval shall be given only by the body or officer assigned authority for that document class.
3.2 No person may publish a document as a CHSPS controlled requirement solely because they authored, edited or administered it.
3.3 Accreditation standards and normative revisions shall follow the published standards-development and public-comment process when stakeholder review is required by that process.
4. Edition, revision and effective dates
4.1 Each approved change shall be assigned a new edition or revision identifier adequate to distinguish it from the prior text.
4.2 The approval date records when the competent authority approved the document. The effective date records when the requirements begin to govern. Those dates may differ to permit implementation notice.
4.3 Substantive requirements shall not be applied retroactively to a completed decision or active matter unless the controlling agreement expressly permits the change or retroactive application is required by law.
5. Publication and authoritative copies
5.1 Controlled documents intended for public use shall be published through an official CHSPS web location or downloadable file and listed in the Master Controlled Document Register or other designated authoritative register.
5.2 The register shall identify the current status and authoritative location. If a website summary and a controlled document conflict, the controlled document governs.
5.3 Downloadable PDFs may consolidate related policies for convenience. A consolidated manual shall identify its source documents and shall not silently change the meaning of the underlying controlled text.
6. Change control
6.1 Proposed changes shall identify the reason for change, affected documents or processes, approval route and implementation needs.
6.2 Editorial corrections that do not alter meaning may be processed through administrative control; substantive changes require the approval level assigned to the document and, when applicable, stakeholder review.
6.3 Related web pages, forms, templates and data registers shall be reviewed for consistency when a controlled document changes.
7. Superseded, withdrawn and archived documents
7.1 A superseded or withdrawn document shall no longer appear as the current authority. The public or internal record shall identify its superseded or withdrawn status where retention is necessary for traceability.
7.2 Archived editions shall remain retrievable for audit, appeal, historical interpretation and determination of which requirements applied to a past matter.
7.3 Destruction of archived records is governed by CHSPS-ACC-104 and any legal hold.
8. Periodic review
8.1 Document owners shall review controlled documents by the stated next-review date or earlier when law, recognized practice, organizational structure, program scope, material incident or stakeholder feedback indicates a need.
8.2 Review may result in reaffirmation, revision, consolidation or withdrawal. The outcome shall be recorded.
9. Access, security and integrity
9.1 Editing rights to authoritative source files and registers shall be restricted to authorized persons. Published files shall be protected against accidental replacement and shall be backed up through the organization’s normal information-management controls.
9.2 A suspected publication error that could materially affect an applicant, accredited organization, stakeholder or decision shall be escalated promptly. CHSPS shall correct the authoritative source and communicate the correction when reliance on the error could cause material harm.
10. Records of control
10.1 Approval records, revision history, source files, public-comment disposition records where applicable, publication evidence and superseded editions shall be retained according to the applicable records schedule.
10.2 The Master Controlled Document Register is the public inventory of current CHSPS controlled publications; it is not itself a substitute for the full text of those documents.
