CHSPS-ACC-101 · Published Policy · Edition 2026.1 Controlled vocabulary

What each CHSPS accreditation decision actually means.

A finding, a decision and an ongoing status are not the same thing. CHSPS uses one controlled vocabulary so the review record, written decision, My CHSPS, public verification, certificates and permitted claims describe the same outcome consistently.

Only a final accreditation decision can create accredited status. Eligibility, applicant, reviewer, education, membership or payment status never does.

Initial accreditation decisions

Four controlled outcomes — with very different consequences

These are the initial decision terms under CHSPS-ACC-101. The written decision controls the exact scope, effective dates, conditions and next steps.

positive

Accredited

All requirements for accreditation have been satisfied for the approved scope.

Public record
Published and verifiable after finalization.
Mark / claim rights
Permitted within the approved scope and subject to the Mark & Claims Policy.
What happens next
The organization enters the applicable maintenance and renewal cycle.
conditional

Accredited — Compliance Report Required

Accreditation is granted, with specifically identified post-decision reporting obligations and deadlines.

Public record
Published as an accredited status with the controlled condition reflected as applicable.
Mark / claim rights
Accredited status is granted; claims and marks remain limited to the approved scope and controlled terms.
What happens next
Submit the required compliance report by the stated deadline while normal maintenance obligations continue.
pending

Deferred

No final accreditation award has been made. The decision body requires specified information, correction or action before completing the decision.

Public record
Not published as an accredited organization.
Mark / claim rights
No accreditation mark or accredited-status claim rights.
What happens next
Complete the specified action within the decision window; the record returns to the controlled decision process.
adverse

Denied

Initial accreditation is not granted. CHSPS provides written reasons and applicable due-process rights.

Public record
Not published as an accredited organization under the ACC-101 publication rule.
Mark / claim rights
No accreditation mark or accredited-status claim rights.
What happens next
Use the applicable reconsideration or appeal route, or reapply after the identified gaps have been addressed.
Ongoing statuses

Status tells you where the record stands now

A status may preserve accreditation, restrict it, end it, or simply show that a decision has not yet occurred. The public record and mark rights must follow the exact current status.

Applicant

An application is active but no accreditation decision has been issued.

Public visibility

Not published as an accredited organization.

Rights / next step

No accreditation mark rights. Continue the active review process in My CHSPS.

Accredited — Renewal Review Pending

A timely, complete renewal was accepted before expiration and the renewal decision remains pending.

Public visibility

The current accreditation remains publicly verifiable under the controlled continuation rule.

Rights / next step

Accreditation remains active unless a separate suspension or withdrawal basis arises. Continue maintenance obligations while the renewal decision is pending.

Suspended

A previously accredited record temporarily loses some or all accreditation rights pending corrective action.

Public visibility

Publicly visible as suspended.

Rights / next step

Rights are restricted or paused according to the written action. Satisfy the corrective-action requirements or other conditions stated in the action.

Withdrawn — for cause

CHSPS terminates accreditation for sustained or serious nonconformity after the applicable process.

Public visibility

The historical/public record preserves the withdrawal and effective date.

Rights / next step

Current accreditation mark rights end. Follow any applicable reconsideration/appeal rights and cease current accreditation claims.

Withdrawn — voluntary

The accredited organization voluntarily surrenders accreditation under the published rules.

Public visibility

Recorded as voluntary in the historical/public record.

Rights / next step

Current accreditation mark rights end. Stop current accreditation claims and follow any record-closeout requirements.

Expired

The published term ended without a timely renewal-pending continuation or successful renewal.

Public visibility

The historical verification record remains.

Rights / next step

Current accreditation mark rights end. A new CHSPS decision is required for reinstatement after expiration.

Findings taxonomy

A finding informs the record; it is not itself the final decision

Technical reviewers document findings against requirements and evidence. The completed technical record then moves to the authorized decision process.

systemic gap

Major nonconformity

Absence or systemic failure of a required control.

Decision effect

Blocks accreditation until corrected and verified.

isolated lapse

Minor nonconformity

An isolated lapse in an otherwise functioning control.

Decision effect

Corrective action is required, but the finding does not automatically determine one universal decision outcome.

risk noted

Observation

The requirement conforms, but a documented risk merits attention.

Decision effect

Tracked; no corrective action is required solely because the observation exists.

good practice

Opportunity for improvement

A non-binding suggestion for stronger practice.

Decision effect

Never determines or changes the accreditation decision by itself.

Why the distinction matters:

An applicant should respond to the exact finding and evidence issue rather than treating every reviewer comment as a denial or resubmitting the entire application.

The written decision

A usable decision should make the record understandable

The controlled decision communication and application record should make the material consequences clear without requiring the organization to infer them from internal workflow codes.

  • OutcomeThe exact controlled decision or status term.
  • Approved scopeThe program, system, service or defined offering to which the decision applies.
  • Basis and rationaleThe material review basis supporting the decision.
  • Conditions and deadlinesAny compliance report, corrective action or other required follow-up.
  • Effective datesWhen the decision and associated rights take effect.
  • Due-process informationApplicable reconsideration or appeal rights and filing route.
  • Public-record next stepHow the finalized outcome is reflected in verification when publication applies.
Corrections, reconsideration & appeal

Use the route that matches the problem

A clerical mistake, a disputed technical conclusion and a procedural challenge are different issues. CHSPS uses separate routes so each is handled by the appropriate independent process.

Administrative correction

Use it when
A factual or clerical error appears in a letter, listing or certificate.
Who handles it
Accreditation administration / record control.
Timing / fee
Any time · no fee.
Possible result
The factual record is corrected without forcing the organization through a substantive appeal process.

Reconsideration

Use it when
Relevant evidence was misunderstood, a factual conclusion needs correction, or material information qualifies for consideration.
Who handles it
Before final decision: review lead plus an independent member. After final decision: Accreditation Decision Committee.
Timing / fee
File within 30 days of findings or decision · free. Target written outcome within 30 days.
Possible result
Conclusion confirmed, revised, or returned for focused re-review.

Appeal

Use it when
You challenge procedural error, due-process failure, conflict of interest, or improper application of published standards.
Who handles it
Independent Appeals Panel with no role in the original review or decision.
Timing / fee
Normally within 30 days of an adverse decision · free. Target written decision within 45 days.
Possible result
Decision upheld, vacated, remanded for reconsideration, or independent re-review required.
Reconsideration does not erase appeal rights.

When a timely reconsideration is requested, the appeal filing period is tolled and begins again when CHSPS issues the reconsideration outcome.

Publication & public trust

The public record follows the final controlled status

Public verification exists so employers, regulators, applicants and the public can distinguish current accreditation from applicant or unresolved review status.

Published / verifiable

Current or historical accreditation records

Accredited records and applicable ongoing public statuses such as suspension, withdrawal and expiration remain tied to the controlled verification history.

Verify Accreditation →
Not published as accreditation

Applicant, deferred and denied applications

These states do not create accredited status and are not presented as accredited organizations under the ACC-101 publication rule.

Separate public register

Adverse and administrative actions

Actions that change or condition a public accreditation status are published under the applicable action and appeal-hold rules.

Need to interpret a status?

Use the official record, not a marketing claim.

Verify the exact accreditation number for current public status. If you are an applicant reviewing a CHSPS decision, use My CHSPS and the written decision as the authoritative case record.
